General Summary/
Responsibilities:
Provides direct and indirect clinical social work and transitions planning services for patients and their families in various inpatient and outpatient settings.
Participates as a professional team member in patient care planning along with other hospital and community agency personnel.
Frequent contact with patient care services staff, physicians, and ancillaries to discuss patient care and diagnostic needs.
Participate in care rounds and other interdisciplinary collaboration within the in-patient and outpatient programs.
Frequent contact with community agencies, third party payers, outside review organizations and case managers.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
